Form G - NFCYM/NCYC Liability Waiver and Permission Form

As a condition of participation in the National Catholic Youth Conference, both chaperones and youth participants must complete the appropriate NFCYM/NCYC Liability Waiver and Permission Form. No changes may be made to the forms. Any change to the form, invalidates it and the chaperone or young person will not be allowed to participate in NCYC. Forms must be notarized.
